"A scroll Lightning Blade" sounds wrong because the nouns are in the wrong order, so I changed it to "A Lightning Blade scroll." Another answer could be "The scroll Lightning Blade," because the scroll and Lightning Blade are two separate nouns.
"Add (x) into" sounds strange. We'd say "add (x) to," even if it is going inside.

@Aoki_Games Yes, they do not have the same meaning. In "The Lightning Blade scroll," "Lightning Blade" is an adjective that describes the noun, which is the scroll.
In "The scroll, Lightning Blade," you are saying "This is the scroll, and its name is Lightning Blade." You also can't say "A scroll, Lightning Blade," here.
They're both very similar to each other, but they are a little different.
